DISCLAIMER: I understand that shelter animals may receive minimal training and basic veterinary care, such as routine vaccinations, while impounded and may require training and further medical care once adopted. I also understand that once adopted, it is then my financial responsibility to seek care for any further medical or behavioral issues the adopted animal may have, whether known or unknown at time of adoption.

I understand that it is my responsibility to see and evaluate the animal for myself before agreeing to the adoption. The adoption of a lifelong pet should not be impulsive or rushed, but rather a carefully thought our decision which will ensure a loving, lasting relationship. I also acknowledge that I have answered all questions honestly.

Falsified information will lead to automatic rejection of the application. The Bloomfield Animal Shelter Management and Staff reserve the right to refuse any applicant.
