The CARE Unit provides police and social worker co-response to scene upon request and follow-up case management to those experiencing crisis, with the goal of building community resilience through linkage to providers for identified needs. Referrals to the CARE Unit are provided through the police department during or after a police contact with a resident where the needs including mental health, homelessness, food insecurity, domestic violence, substance use, and senior specific needs are identified. A social worker will then provide outreach, including benefit screenings, safety, mental status, and psychosocial assessments to identify needs and create a person-centered care place with linkage to services.
